This has been used approximately 5 times and is roughly 3 months old. This was on an extra line in our garage, but we recently got a Tesla that will be using this line all the time, so I swapped this for a Tesla EVSE. Everything works as it should.

This is the 48A model, so it only came with the pigtail for hardwire only.

ah! ok I only have a 14-50 nema installed and would need an electrician to come out and do the hardwire. I'll think on it.
